Cornerstone VNA Expands to Serve Southern NH Seacoast Towns

Cornerstone VNA, a local nonprofit health care provider, is proud to announce the expansion of its service area to include the Seacoast towns of Stratham, Hampton, North Hampton, Hampton Falls and Seabrook. With the addition of these five communities, Cornerstone VNA now offers home care, hospice, palliative, private duty and community services to over 40 towns in New Hampshire and Maine. According to the organization’s leadership, the expansion meets the demand of local health care providers who wanted to offer additional home care service options to their patients. It also addresses the growing needs of the aging population in the area.

The Team at Cornerstone VNA offers specialized care at home and advanced technology to provide highly skilled nursing, rehabilitative therapies, social work, personal care and more to patients of all ages. Cornerstone VNA provides a full range of services through its five distinct programs including Home Care, Hospice Care, Palliative Care, Life Care-Private Duty, and Community Care. These services are designed to improve the quality of life for patients and to help them live safely and independently in the comfort of their home.

According to Cornerstone VNA President/CEO, Julie Reynolds, RN, MS, “Over the years, there has been a significant increase in demand for our services, and we are excited to welcome these new towns to our service area.” She adds, “Many people want to remain at home as long as possible, and I am grateful to our team who provide such wonderful care and support to help our patients, families and caregivers achieve this goal.”

In addition to the programs offered by Cornerstone VNA, residents in these five towns will now have access to free educational programming, volunteer companion services, and family caregiver support. Also, the expanded Palliative Care program at Cornerstone VNA now includes a physician and several nurse practitioners who provide expert care and symptom management to patients living with chronic illnesses. Through an innovative and award-winning Telehealth program, Cornerstone VNA is pleased to offer remote monitoring to high-risk patients, which improves quality of life and reduces the number of emergency room visits and re-hospitalizations.
