NH Care Connections to Host Community Event at Frisbie Memorial Hospital on 11/20

NH Care Connections is inviting local organizations and community members to an in-person event on Wednesday, November 20, 2024, at Frisbie Memorial Hospital, located at 11 Whitehall Road, Rochester, NH. The event will take place from 3:30 PM to 5:00 PM.

The session is part of a statewide series aimed at strengthening partnerships between healthcare, housing, employment, and other community service providers. Attendees will have the opportunity to meet the NH Care Connections team, learn about the benefits of joining the network, and explore ways to enhance coordinated care for residents in Strafford County and the Seacoast region.

NH Care Connections is a closed-loop referral network designed to streamline communication between providers, reduce redundancies, and improve overall health outcomes. By bringing together various services, the initiative aims to ensure better experiences and support for patients in need.
